Hybrid ECMO in Adult Septic Shock with ARDS
Septic shock in adults remained a pandora’s box as per the indication of ECMO.
Recently, we encountered a 65-year-old male patient admitted with community-acquired pneumonia with ARDS with hemodynamic compromise. Even with maximum ventilatory settings with an inotrope score of more than 100, he remained unstable and showed features of organ dysfunction.
He has been rescued by primary VAV ECMO with Femoro-Jugular cannulation. As his hemodynamics started improving with lowering vasopressors, the arterial support took off and converted to VV ECMO on Day 6. As pneumonia got resolved and Oxygenation started improving, his VV ECMO weaned off on day 14.
His tracheostomy got decannulated and discharged on day 25 in stable condition.
Primary hybrid ECMO showed a light at the end of the tunnel in such serious and much-debated adult septic shock.
